In order to get this trophy, you must have traveled quite some distance on Grand Pulse (just moving directly to your destination in Chapter 11 most likely won't do), but you also must repair and afterwards talk to Bhakti in Oerba to actually unlock it.
You can get this trophy in chapter 11 by exploring any area that makes up Grand Pulse and by solving those Cie'th stone missions you can do without too many problems as that will add to your distance traveled.

Once you talk to Bhakti you are going to be rewarded with three platinum ingots when the condition is met - and the trophy unlocks shortly thereafter.
When you didn't get the platinum ingots after having talked to Bhakti, just explore a bit more and return some time later.

In case you returned to Cocoon before you obtained this trophy, you can attempt to get it once you find a way back to Grand Pulse in chapter 13 or after finishing the main story line.

Best thing is to maximed everybody in all Paradigms and have Tier 3 Weapons for the ATB 1, then use Fang as leader to summon Bahumut and beat the legs. DeBuff it as much as possibile, then build Stagger to 999% with Ravagers. Once it reaches the maximum, launch three Commandos (Cerberus Paradigm) to waste him. I had to use an Elixir to re-summon Bahamut.

No offence to the person that wrote it but the tip by Myster See is incorrect. The trophy is NOT missable because it says "Held every weapon and accessory" Instead of "Hold every weapon and accessory"

You don't need to hold them all at the same time, holding them once is enough. If you sold or upgraded things they still count for the trophy. So if you buy a level 1 weapon and upgrade it to level 2 you don't need to buy another level 1 because you already had it once.
I did it this way and I'm 100% sure having them once is enough.

This trophy only refers to the third form of the boss battle and not all three battles. The time you've been givin to 5-star this battle depends on your attack and magic score: the higher these are, the shorter the battle can continue. The Gold Watch accessorie extends the time and is a must to 5-star this battle. You will receive this after completing mark 64 (defeat the Undying Vercingetorix) on Gran Pulse, so maxing out all characters first is a wise choice.
