Play from the very first level to the first real boss... the big guy with the spike-y block thing that he slams down at you... without hitting anyone. Sounds impossible right? Well, it is. Alone. Play with some friends (or use a second controller), let them do all the work and never press any of the attack buttons. Go without hitting anyone until after the boss is beaten and you'll have your trophy. Easy peasey.
