This trophy (like 
Faerûnian Idol and 
This Belongs in a Museum) is obtained through one of the three possible outcomes of excavating the ruins at the Archaeological site. Save the game before going for it so that you can reload and make the other choice.
In the area southeast of the lighthouse, there are ruins roughly in the centre of the map. Agree to help with the excavation but then accept Gallor's offer to double cross. Once inside, kill the mad diggers and then betray and kill Nib. Remove the item from the dig and leave. Kill the insane diggers, give the relic to Gallor and you will get the trophy.
